Candidate must be a Third Degree member in good standing. There is no longer a six (6) month
minimum after making their First Degree.
Candidates must have an approved Form 4 (if possible use the online form) along with $70.00 application fee submitted to your Local Comptroller
If your candidate is a Priest the cost is $60.00 and is charged to your Local Assembly
Comptroller two weeks before Exemplification date to be considered for the Exemplification of the Fourth Degree.
Although exceptions are sometimes made.

He in turn will submit your money and form to the District Comptroller.
· Priests and Brothers application are the responsibility of the Sponsoring Assembly.

Dress Code
Dress code for candidates and members is a minimum of a dark suit,
 with long tie and black socks and shoes or the new official 4th Degree Uniform.
 Black tuxedo (basic style, no double breasted or tails, no colored accessories) is now optional.
The only exceptions are priests and members of the military or first responders in their dress uniforms.
